Lock:                      Has a crack on it refer to the images

Lens/Membrane:  No nick or scuff observed on it but seems to be swollen a bit , refer to the                                 images

Scan Analysis:

Air Scan:            Smooth, no shadows have been observed, refer to the images

Phantom Scan:  Smooth, refer to the images.

Body Scan:         Review the Hepato Renal Scan and Carotid Renal Scan for blood flow has been                        imaged in image 4 and you may exercise your judgement
